Becoming an SEFBHN Network Provider

SEFBHN welcomes community agencies with an interest in becoming SEFBHN providers to complete our SEFBHN Qualified Provider Application (see link below) and email it to Tracee.Diaz@sefbhn.org. Please note that decisions regarding adding new providers to our network is dependent on a range of factors.
