Care Coordination: Specially-trained Care Coordinators help connect people with multiple services they may need in the community, providing direct referrals, warm handoffs between different agencies providing services, and ensuring coordination between those different services, especially when a person is transitioning from one service/agency to another. This services is different from Case Management, please see definition of Case Management below.

Case Management: A Case Manager helps people, usually those who are already receiving other services at an agency (such as therapy), achieve their personal goals by helping them obtain a variety of services and assistance. This can include help with getting financial assistance (food stamps, medicaid, etc.), housing, financial planning, employment assistance (job training, etc.), other treatment services, and more.
